Using Online Forms on the National Air and Space Museum Web Site
Some fields in almost every form will be required, so please read each form carefully to see which fields are required. If you do not fill in each of the required fields, the form will not submit and you will be instructed to check for missed fields. Please be sure to check your information for accuracy before submitting the form. Forms sometimes take a few seconds to process, so please click the submit button only once.
Once you have submitted the form, you should receive a confirmation screen telling you that you have successfully submitted the form, and often telling you exactly what information you sent to us. If you are expecting a specific action to be taken based on your form submission, please print or save the confirmation page for your records.
Most forms on this web site send your submitted information to the responsible person(s) at the Museum, and it is up to Museum staff to contact you to fulfill your request. Museum staff will always try to contact you as soon as possible by phone or by email, but please be patient while we process your requests. Some forms on this site will perform automated actions on your requests in which case you will be told what to expect and what actions have been taken, on the form itself, or on the resulting confirmation screen.
