The CAP's Distinguished Service Medal is the third highest decoration possible in the CAP and is awarded for conspicuous performance of outstanding service in a duty of great responsibility. It is usually awarded for completion of an extremely difficult duty of marked national significance.

Civil Air Patrol (CAP) Distinguished Service Medal; white background, blue rectangle in center adorned by two gold bars at the 1/6th and 5/6ths marks on the rectangle; ends of ribbon consist of three bars, being a large white, medium red, and medium white from the outside in.
