Various prepared notebooks and checklists were provided for astronaut reference during the Apollo moon landing missions. This checklist would have been used during the historic Apollo 11 mission if a deviation from the original flight plan had been required.

The checklist was transferred from NASA to the Smithsonian with the contents of the Command Module "Columbia" in 1970.
